In order to be considered as an adopter, you must:

·         Be at least 18 years old and have photo ID showing present address.

·         Have verifiable consent from your landlord if you are a renter.

·         Have a carrier to take the adopted cat home.

In an effort to ensure a satisfactory adoption, your cooperation of this agreement is necessary.  The welfare of the animals and the adoptive families is at stake.  Therefore, we reserve the right to refuse any adoption we consider unsatisfactory if a particular pet you selected is deemed not suited for your particular situation.  Name_________________________________County___________________________

Adoption is a lifelong commitment. 

Please read, then write your initials next to each item below to confirm your understanding and compliance. I  agree: 

1.        ___To have time to care for a cat.  I can provide litter box cleaning, feeding, grooming and daily attention.

2.        ___To have considered the cost of owning a cat.  I can provide veterinary care, food, grooming and the proper amount of space.

3.         ___Allergies can be a significant problem for some individuals.  I understand visiting friends or family may be in serious discomfort. This is not a problem for me or my family.

4.         ___To have thought this adoption through.  After the novelty wears off I know I will be dealing with training, pet hair, vet care, food, grooming, proper space and cat litter. 

5.        ___To work training the cat to stay off of cabinets, tables, etc. and exercise great patience.  I will use a spray mist bottle (not in the face) or shake an empty soda bottle with coins.  I understand that if I am consistent, it will change the behavior eventually. 

6.        ___To keep the adopted animal exclusively indoors.

7.         ___For the cat’s safety, I agree that the cat will be carried in a cat carrier any time it is transported.

8.         ___A $25 deposit will be required at the time of adoption and will be returned to you after spay/neuter is completed and documented.  This only pertains to kittens not old enough (younger than 5 months) at adoption time. 

9.        ___To establish a relationship with a licensed veterinarian within 2 weeks of this adoption. 

10.     ___To provide veterinary care to prevent and cure illness. 

11.     ___To accept the cat as is.  Feline Finish Line is vigilant about the care of our cats and is not responsible for any unforeseen medical condition that might occur.  Since we house stray and unwanted animals, we cannot guarantee their health.  However, every effort has been made to ensure that healthy animals are available.

12.     ___ If the cat is seriously ill within the 1st 24 hours and you need a vet, the cat/kitten must be taken to our official vet, Dr. Karen Hess.  Call us immediately. Feline Finish Line will not be responsible for payment to another vet.

13.      ___To not sell, give away, abandon or use the animal for experimental purposes.

14.     ___ I hereby agree that the adoption fee is not refundable; however I may return the cat/kitten or make 1 exchange of the cat/kitten within 30 days of adoption if for any reason I cannot keep the adopted pet.

15.     ___To notify Feline Finish Line of any change in address or telephone number.

16.     ___To provide access to the animal at the request of Feline Finish Line representatives who may visit to check on the animal’s welfare.  Feline Finish Line will provide a follow up call within 2 weeks after adoption to discuss any concerns you might have and see how your new pet is doing.

17.     ___To pay Feline Finish Line up to, and including, $300.00 if any of the above terms and conditions of this agreement are breeched and to return the pet to Feline Finish Line.  This is in consideration of services rendered by Feline Finish Line and is not a penalty fine.

18.     ___That I have never been cited by police or health officials for an animal related problem.

Cats are protected under PA law.  PA crimes code:  Section 5511 [C] states that a person commits a summary offense if he wantonly or cruelly ill treats or neglects any animal as to which he has duty or care.  Or abandons any animal or deprives any animal of necessary sustenance, drink, shelter or veterinary care.  Or access to clean and sanitary shelter which will protect the animal against inclement weather and preserve the animal’s body heat and keep it dry.

 

I understand the above PA crimes code Section 5511 [C] and swear that I will be a responsible guardian of this animal.  In the event I am unable to care for this pet, I will relinquish custody of said pet to a no kill shelter for the purpose of placing the cat in another safe home.  This agreement will safeguard the cat from being removed from the shelter of this agreement signed on month_____________ day ______________ 2009.
